Kindergarten Registration

2019-2020 Kindergarten Registration
*Please be sure to read all of the information below!*

The 2019-2020 Kindergarten Registration for South Park Elementary Center has begun. In order for your child to be eligible for kindergarten, the student must reach the age of five (5) before September 1, 2019. All kindergarten sessions are full-day sessions beginning in the 2019-2020 school year.

Registering for kindergarten is as easy as 1, 2, 3!

STEP ONE:
Complete the online pre-registration form. Click here to access the online form.

STEP TWO:
Schedule a kindergarten screening assessment for your child. This can be done online by clicking here. Screening assessments will be conducted on April 8 and 9. Please arrive 5-10 minutes before your child’s scheduled time. The kindergarten screening assessment is a simple 10-15 minute assessment to check basic kindergarten readiness. Skills such as letter and number identification, rhyming, counting, saying the alphabet and shape identification are some of the basic skills students may have before entering kindergarten.

STEP THREE:
Download a registration packet by clicking here or stop by the elementary center office from 10 a.m. to 2:30 p.m. on weekdays to pick up a packet. Completed registration packets need to be returned on the day of your child’s screening assessment.

Your child must have received the following vaccinations before entering school:

(all dosing and spacing requirements set forth by the PA Department of health must be followed)

  • 4 doses of DTP (1 dose must be on or after the 4th birthday)
  • 4 doses of the Polio vaccine (1 dose must be on or after the 4th birthday)
  • 2 doses of MMR (1st dose must be after the 1st birthday)
  • 3 doses of the Hepatitis B vaccine
  • 2 doses of the Varicella Vaccine (1st dose must be after the first birthday) or a written statement from a physician indicating month and year of disease or serologic proof of immunity.

Allegheny County Health Department Universal Blood Lead Level Testing:  All students entering Kindergarten will be required to receive a blood lead level test.  Each child shall be tested between 9 and 12 months and again at approximately 24 months.  If the child did not have their blood tested at those times, the child is to have the test done as soon as possible but before entrance to Kindergarten.

All students entering school will also be required to have a physical and dental exam dated within one year of entering school. If you are going to the doctor prior to registration you can find the medical forms to be completed for your child’s physical/dental exam on our web site at www.sparksd.org and by clicking on Health Services under the Departments tab.

You will also need a birth certificate for registration.
